Sew Knit Collars

Knitting a collar help.?
Ok, I need some help here. I have stitches on each side of a cardigan on a stitch holder and the sides are sewn to the back . The pattern says to pick up the stitches from the stitch holder, join the yarn, pick up 20 along the back, then pick up the remaining stitches from the holder on the other side. After picking up the stitches on the back part of the sweater, how do you get the yarn to the top of the work when you take the remaining stitches off of the stitch holder?? I know this may sound stupid, but I can not figure it out.
I'm not sure why your yarn wouldn't already be at the top of the work. You should be going straight across the top of the sweater. from right to left.
Depending on whether you started from the right or wrong side of the sweater, you'd attach your yarn and knit the stitches on the stitch holder on your right, then keep going and knit the ones on the back, then the ones on the left stitch holder.
You could also assemble all the stiches, first from the left stitch holder, then the back, then the right stitch holder, onto one needle and knit as usual.
